To begin the evaluation and therapeutic process, please call or email Caroline Millan, our Office Manager. Caroline will collect your contact information and a brief description of your concerns (the kids we see). One of our two clinical Directors, Sherri Cawn M.A., CCC-SLP or Jerri Krantz M.A., CCC-SLP, will review your information and communicate with you within 48 hours of your initial request. This conversation enables you to review all pertinent information about your child with a director and discuss your concerns in depth.

After the initial consultation with a Cawn/Krantz director is completed, new clients are required to complete paperwork which includes a health history form and information about fees and payment. These materials help gather important health information about your child and prepare him/her for a speech-language, occupational therapy evaluation, or feeding evaluation. We ask that all Intake Packet paperwork be completed and returned to the clinic before we schedule your first appointment. You may return these documents via fax, email, mail, or clinic drop-off. You will be able to fill out the forms on-line and save to your computer, but due to HIPPA regulations you must print it out before sending them to us.

The cost of an evaluation at Cawn/Krantz depends on the complexity of the evaluation. Consistent with this approach, the fee may be a combination of the evaluation and formal testing by the therapists, the medical decision-making, and the time spent developing an individualized treatment program for the child.

Payment is due at the conclusion of the evaluation, unless prior arrangements have been made with the Office Manager. Cash, check, or credit card (VISA, MasterCard, Discover & American Express) are accepted.  We are In-Network Providers with Blue Cross/Blue Shield and Humana.

Cawn/Krantz participates in the Blue Cross/Blue Shield and Humana network. For clients with other insurance plans, it is the patient's responsibility to submit a copy of an itemized bill of services to his/her individual insurance carrier for evaluation of benefits.

For prospective patients interested in a predetermination of benefits with their insurance company, please download this Insurance Information Letter example to see the typical fees associated with both an initial evaluation and therapeutic treatment sessions at Cawn/Krantz & Assoc., Ltd. Developmental Therapies. The Insurance Information Letter is meant for insurance predetermination purposes only; actual charges will be based on the services provided.
