How to Become a Client at Cawn/Krantz & Assoc., LTD. Developmental Therapies

To begin the evaluation and therapeutic process, please call or email Caroline Millan, Office Manager of Cawn/Krantz & Assoc., Ltd. Developmental Therapies. Caroline will first collect your contact information and a brief description of your concerns (click here for the kinds of clients we see). One of our two clinical Directors, Sherri Cawn (M.A., CCC-SLP/L) or Jerri Krantz (M.A., CCC-SLP/L), will review your information and communicate with you within 48 hours of your initial solicitation. This conversation enables you to review all pertinent information about your child with a director and discuss your concerns in depth.
After the initial consultation with a Cawn/Krantz director is completed, new clients are required to complete an Intake Packet; a health history form and information about fees and payment are included in this packet. These materials help gather important health information about your child and prepare him/her for a speech-language or occupational therapy evaluation. We ask that all Intake Packet paperwork be completed and returned to the clinic before we schedule your first appointment. You may return these documents via fax, email, mail, or clinic drop-off.
FEES AND PAYMENT POLICY
The cost of an evaluation at Cawn/Krantz depends on the complexity of the evaluation. Consistent with this approach, the fee may be a combination of the evaluation and formal testing by the therapists, the medical decision-making, and the time spent developing an individualized treatment program for the child.
FINANCIAL POLICY
Payment is due at the conclusion of the evaluation, unless prior arrangements have been made with the Office Manager. Cash, check, or credit card (VISA, MasterCard & American Express) are accepted.
Once the initial evaluation process is complete, Cawn/Krantz clients are billed monthly for therapeutic sessions. This monthly statement is due upon receipt.
INSURANCE INFORMATION
We do not participate in any health insurances, health insurance networks, Medicaid, or Medicare; we cannot accept assignment for insurance. It is the patient's responsibility to submit a copy of an itemized bill of services to his/her individual insurance carrier for evaluation of benefits.
